Level 1 Use of Workshop Materials
I acknowledge that after attending the PSYCH-K® Online Workshop-Level 1, I have the right to use the Workshop Training Materials with myself, exclusively. Further, while I have the right to use Workshop Training Materials with myself, as outlined above, I agree not to use/Facilitate PSYCH-K® with others. I acknowledge and understand that the PSYCH-K® Online Workshop-Level 1, does not provide training to facilitate PSYCH-K® with others. I agree not to use/Facilitate PSYCH-K® with others in-person, unless I have attended one or more of the in-person workshops, then in-person sessions can be facilitated. Furthermore, I agree not to teach the PSYCH-K® processes to others in any way, to groups or individuals, professionally or otherwise, for monetary gain or otherwise. I further agree not to post any video presentations of PSYCH-K® processes, or demonstrations on the internet, or other electronic media.

The PSYCH-K® process is strictly limited to the modification of beliefs. The PSYCH-K® process is not designed to diagnose medical conditions, nor is it a replacement for medical attention or professional mental health care.
Copyrights and Trademarks
I acknowledge that The Myrddin Corporation (d.b.a. The PSYCH-K® Centre International) is the sole owner, and/or the exclusive licensee of certain rights, including but not limited to copyrights, trademarks, concepts, and confidential trade secrets, workbooks, handouts and materials pertaining to the PSYCH-K® program. I further acknowledge and agree that the PSYCH-K® training materials, logos and other trade symbols used in PSYCH-K® Workshops and on the PSYCH-K® website (www.psych-k.com) are proprietary, intellectual property of The Myrddin Corporation, and I agree not to reproduce, in whole or in part, or to utilize, plagiarize or imitate any of the PSYCH-K® concepts, technology, and strategies, or the handouts, materials, or Website graphics, in any manner, without the express written consent of The Myrddin Corporation. Furthermore, I agree not to record, in whole or in part, in any manner, including audio, video, or photographs, any part of the instructional or experiential portions of the online Workshops. Further, I agree not to distribute any of the Workshop training materials used in the PSYCH-K® Online Workshops to anyone, in physical, digital or photographic form.
Post Level 1: I understand I may not use the PSYCH-K® name (whether upper or lower case, letters) as part of a website domain name or URL, or as part of an email address, or as a part of a social network profile and/or page name. In addition, I also agree not use the Falcon logo, or the Myrddin Corporation trademark spiral logo, displayed on the PSYCH-K® Centre International website, for commercial or other purposes without the express written permission of The Myrddin Corporation.
I agree to be bound by, and will comply with all the conditions of this Agreement. I agree that I will not be informed about the acceptance of this Agreement by The Myrddin Corporation. Further, I agree that this form be forwarded to and kept by my contractual partner, The Myrddin Corporation, (d.b.a. The PSYCH-K® Centre International), P.O. Box 1885, El Prado, New Mexico, 87529 USA, that may store it electronically, using a third-party IT service.
Level 2 Facilitation
I acknowledge that after attending the PSYCH-K® Online Workshop-Level 2, I have the right to use the PSYCH-K® processes in private one-to-one sessions, only via online or audio, either professionally or personally, with clients or others, for monetary gain or otherwise, at my discretion. I agree not to use/Facilitate PSYCH-K® with others in-person, unless I have attended one or more of the in-person workshops, then in-person sessions can be facilitated. Furthermore, I am authorized to use the title of PSYCH-K® Facilitator to describe my status with respect to my use of PSYCH-K®, in marketing or other printed materials. This designation does not imply Certification of any kind in PSYCH-K®. the designation as Certified is reserved for Certified PSYCH-K® Instructors, only.
Post Level 2: I understand that I may use the PSYCH-K® name on my website, business card or other printed materials, provided I use all capital letters and include the registered trademark symbol (®). I may not use the PSYCH-K® name (whether in upper or lower case, letters) as part of a website domain name or URL, or as part of an email address, or as a part of a social network profile and/or page name. I may use the Falcon logo, (Above right, available to PSYCH-K® Facilitators) but the spirals logo (above left) may be used only by The Myrddin Corporation or its Certified Instructors.
Be advised that the Falcon logo must be used as it appears, without alteration, in one of the three authorized colors (black, white, or teal) as a stand-alone symbol. In addition, you may change the size of the logo to fit a specific format. Upon completion of the workshop, you will be provided a link to download this resource.

The storing of this information is for internal purposes only, and is required to complete and activate this contract.
